Useful information before you visit us

On arrival at our practice, you will be asked to complete a Patient Information Form to help us record your details more efficiently. To save time you can fill this out before arriving.

If your examination relates to a Workers Compensation Claim or Third Party Insurance Claim, your doctor will be asked to complete a Workers Compensation / Third Party Claim Information Form. If you know which form you need you can fill this out before arriving to save time.

Radiology Group can deliver your results online though our patient portal. A link to create an online account will be sent to your mobile or email when you attend your next appointment.
